The Massachusetts Healthcare Safety and Quality Consortium convenes health care organizations, patients, payers, policymakers and others to develop and oversee a strategy and action plan for achieving major gains in the safety and quality of care delivered in all care settings throughout the Commonwealth.

The Challenge of Safe, Quality Health Care

Great progress has been made over the past 25 years to improve the safety and quality of health care, yet preventable harm persists in all care settings. Recent Betsy Lehman Center research found almost 62,000 preventable patient harm events resulting in over $617 million in excess health care costs in a single year in Massachusetts and documented the long-lasting physical, emotional, and financial impacts of medical error on patients and families. Other research has shown the negative effects of adverse events and workplace stress and violence on clinicians and staff.  

A Roadmap to Safety for the Commonwealth


Major breakthroughs in safety improvement are within reach but will require sustained, coordinated action. The Massachusetts Healthcare Safety and Quality Consortium is an unprecedented undertaking that brings together the state’s health care community — providers, payers, patients, government agencies, and other policymakers and experts — to accelerate safety and quality improvement statewide. The Consortium is developing a Roadmap to Healthcare Safety and Quality — a strategy and action plan that sets ambitious yet attainable goals to dramatically reduce the risk of preventable harm anywhere health care is delivered throughout the Commonwealth.

The Consortium organizes its work around four “pillars” of safety and quality:

  1. Leadership and governance
  2. Continuous organization learning and improvement
  3. Supporting patients, families, clinician and staff
  4. Measurement and transparency
