New format available to simplify, bolster PFAC annual reporting

The Betsy Lehman Center is offering a fresh approach to hospital-based Patient and Family Advisory Councils’ annual reporting obligation. 

Working with PFAC members across the state, the Center simplified the template most councils use to describe and publicize their activities and accomplishments.  

The 2025 template is available here for use by hospital PFACs in advance of the October 1st deadline for publishing annual reports. 

Primary changes to the existing report template, which has been in use for more than 10 years, include more flexibility to describe key goals, accomplishments and impact, as well as a simplified format for describing the PFAC’s structure, meetings, and membership. 

“This was a group effort and I’m grateful to the PFAC members who responded so thoughtfully to our outreach as we revised the template,” says Jane Powers, Director of PFAC Programs for the Betsy Lehman Center. “We anticipate substantial gains in the ability to share information from PFACs across the state with this new approach.” 

The 2025 template will allow the Center to more easily produce an annual aggregated report to share, generating a more complete picture of PFACs’ accomplishments and challenges across the state. 

The Center also intends to generate professionally formatted summaries of the information submitted by PFACs so they can more readily distill key elements of their work for leaders, patients and others at the hospitals they advise.
