The federal Agency for Healthcare Research and Quality has released the first chart book of hospital patient safety data compiled from Patient Safety Organizations across the country. PSOs began submitting data to AHRQ in 2012, with a major increase in volume of reports in 2016. Here is just one chart from Network of Patient Safety Databases Chartbook, 2019, which reflects analysis of where breakdowns occurred in the process of providing medication and other substances to patients.
